I haven’t done a through review of all the points that make up the WCAG 2.0 guidelines with your plugin but there are 3 things that are glaring to me that should to be addressed.

Tabbing through the message in thread mode does not go to “Toggle Messages” therefore I can’t open any of the closed message that are default.

There is no use of heading tags, h1 -> h6. I do agree that you can’t control which headings are in use at the theme level and in the plugin level.

The images for the users do not have any alt tags indicating they are user photos.

Please see the uploaded mp4 to illustrate the tabbing problem. All I’m doing is hitting the tab key to go through the page.
