    Hi,

    Can you create a customized email template with check boxes and radio buttons as shown in the pictures enclosed?
    how much will charge for this?

    What information you want to show as checkbox and radio button?

    I have gone through the instruction both for my local installation and my online one, but I can’t figure out how to configure it. also, for my online installation, in the pro/template/emails/ folder there is only one template as default.php.

    Please advise,
